How do you approach hard math problems and correct them?

How do you approach hard problems and correct them?

Seriously, on the math tests, I always mess up. For example, in a grid-in I was doing today, I was doing it the right method, but unfortunately got the wrong answer. I cannot say it's because of "not being careful", because I try my best to be careful, and sometimes I think I am (Actually, I don't know what's my problem. I may not be being careful enough, I don't know. I do my best to be careful, regardless of any problem, whether it be easy or hard, and usually, there will always be some problems wrong). And after I check my answers, I immediately try my best to know why the answer is wrong, and how to do it, yet when I do a different section, I get the same amount of questions (i.e: 13/18, 15/20, 12/16 wrong); does this have to do with how I approach a problem?

Give an example and I'll try to help you out
