I am pretty confused on how to fill out the College Report form and submitting it as a transfer prospect. Did any of you go through this already? In my mind I have to download the form, print it out, fill it out, sign it then give it to my teacher to do the rest. I tried to see if I could e-sign the form, but I wasn’t able too. I am assuming after my professor fill out the form they have to re-scan the document, and then email it out to the schools I am applying to. I wish I can just e-sign the form and just forward it to my professors, so they can do the same. Can some one please explain to me how this process works?
just submit a updated transcript from your school. I was asked to submit a mid year report also this morning.
hmm…idk if the answer above is accurate based on your specific question. @inkplay are there certain schools requesting a college report? I 'll share with you below how the process went for me but I can’t be certain this applies to all schools. Process was acceptable for Duke, Stanford, Vanderbilt and University of Richmond if this helps.
-filled out my portion of college report
-made a counseling appointment, initial counselor was fairly clueless but did get the report into the hands of someone with more experience who could help
-second counselor filled out the college report and emailed me a PDF copy within two days, also had admissions dept snail mail report copies plus sent individual emails of the PDF to each university with my full name and DOB listed
A professor filling out the form may not work. The college report I used made it clear that it had to be a school official with, “access to your academic record and disciplinary record”.

@ogdae99 now I have questions Is this due to the current health crisis and potential lack of access to a counseling office? Is it acceptable for U of M specifically and is that the only college OP needs a college report?
If I could have simply submitted an updated transcript that would have made life a lot easier but I knew it wasn’t acceptable for at least two of the schools after contacting admissions so didn’t even bother inquiring if that would work for the others.
I used Adobe Acrobat to fill out my portion. Then I just email the PDF to my transfer advisor. Hopefully she knows how to fill the rest of it out. This coronavirus thing is giving me anxiety, my school is currently open during limited hours. Its going to be a pain in the ass trying to get all the documentations needed. My school doesn’t do email transcripts so everything have to be send out to a physical address.
@inkplay a transfer advisor should be perfectly fine, if she’s not familiar with the process I’m sure she’ll find a way to have it taken care of. There is sometimes a small grace period between an application deadline and documents required. Colleges may be even more understanding about documents given the health crisis. As long as your application was submitted on time you should be ok. Just stay on top of whats needed in your portal. Good luck!
