Do you do it through the common app site? If so, how do you do it? It says it requires the person to be a teacher or counselor. Or does the optional evaluator have to send in the letter physically through mail?

I have the same question...??

bump.................
i just looked at the Stanford FAQs, and it provides no useful information regarding sending the optional letter.

there used to be a special form, in edition to teacher's evaluations ones.

^^^
can that be found in the "school forms" section of the common app website?

My son asked his boss for a letter of rec. The boss wrote the rec on letterhead and sent it via USPS

There used to be a special form?
How do we get access to that?
Is there any sort of "stanford customer support" telephone number? lol.

I guess you no longer need a special form:

5. What about additional evaluators?

You may submit one additional letter of evaluation if you have another person, not a teacher or counselor, who knows you well and will provide us with information about you that your other recommenders will not. We ask that you instruct your additional recommender to place your full name--as listed on your application, your birth date and your current high school at the top of the recommendation letter. Please remember that a letter from a famous person or Stanford alumnus is not going to help us reach a decision if that person is unable to add new insights to your application.
