Well, if you’re applying to undergrad you wouldn’t be applying to HMS, you’d be applying to Harvard College.
Purely based on USNWR rankings, it would go:
1)Harvard
2)Duke
3) UPenn
4) John Hopkins
However, as many people on CC will tell you, rankings are not the whole story. What is important to you in a school? A different, more personal and more accurate ranking will emerge from there.
You do realize that you need a bachelor’s degree before applying to any medical school, right?
You could also rank them alphabetically or by the oldest to newest.

@yonceonhismouth Duke is not higher than Penn on USNWR. Neither for college nor for med school.

For medical school my ranking is:
Harvard
JHU/Penn
Duke
Harvard,JHU, Penn have been consistently in the top 3 med schools throughout the years both in rankings and general perception.
However to get to med school you need to go to college first.
For college my ranking is:
Harvard
Penn
Duke
JHU
I base the college ranking on overall strengths of the departments, desirability by students, yield rates, outcomes and just overall perception as I see it.
@Penn95 actually my mistake- they are tied on USNWR but Duke is the first one written in the #8 category.
^yeah true that is because they are in alphabetical order
They should be ranked correctly, lest you apply to the least of them by mistake.
Getting into even one of those is a major achievement.
