How strong of a Stanford applicant am I? Duke? Vandy?

A huge thanks to those of you who take the time to read and consider this:

So I'm a junior in high school trying to pave the way for top notch college admissions and I have a very unique resume I'd like the users of CollegeConfidential to evaluate, critique and give me advice on that will be beneficial to me come a year from now when I will be applying to colleges. A few of these credentials I've already earned, and some are what I expect to have going into next year.

SAT I: 2240 > (720CR / 800M / 720 W)
ACT: 34 > (E 33 M 36 R 34 S 34 E 10)
SAT II: Chemistry 760, Math II 800, Biology 760
GPA: 3.8/4.0 (read later in essay for explaination)
Class Rank: Top 5%
Junior Schedule: AP Biology, AP Chemistry, AP Language, AP Literature, AP US History, AP Calculus AB (I expect to get all 5's maybe one 4 on the exams) DE College Spanish
Senior Schedule: AP Physics B, AP Calculus BC, AP Psychology, AP US Gov, AP Macroeconomics, AP Enviornmental Science
Previous AP's: 5 on AP Human Geo, 5 on AP World History
I should be a National Merit Finalist, I thought I did really well on the PSAT

Now to the subjective things:

Extracarriculars: Class President (10, 11, 12), JV Football Team (9), Varsity Football (10,11, Senior Captain 12) (I'm good enough to play for an Ivy League school or DIAA school and walk on at Stanford, but not good enough to go there on scholarship. - Same for Vandy and Duke.) NHS member (10, 11, 12) , SNHS member (11, 12), Mu Alpha Theta member (11, 12 - President).

Work Experience: Paid math tutor (10,11,12) , worked at a print shop during springs summers (9,10,11,12)

Essays: Have the potential to be very good.

I fought epilepsy from the time I was born until 9th grade, which is why I had 5 B's and 1 C semester grades during my freshman year when I missed ~40 days of school which has brought down my GPA. Since then, I have made and will keep making straight A's. I would point out that as hellish as my middle school and freshman year was going in and out of hospitals and seizing up during school, I wouldn't trade it for anyone else's past and have come to love it as what has made me who I am today.

My source for my ambition is being told I can't do something. Freshman year, the same teacher that "gave" me the C, recommended me for Standard English the next year and told me that I should stop wearing Stanford shirts because I wasn't Stanford material. That should make an interesting app I hope.

Finally, the well rounded approach - I take great pride in being the guy who can pull off straight A's during the week, play hard and well on Friday night, party and have fun on Saturday night and tutor all of sunday.

Teacher Recommendations: They'll be superb. I have 3 teachers who will write me stellar recommendations.
Personal Recommendations: Will also be great.
Counselor Recommendation: I'm at a public school and we're not that close with our counselors so I'm not sure how that would work.

Hooks: None

State: Florida

Race: White

Gender: Male

HS Type: Public (ranked 13th public school in FL - highest purely public school when magnet and charter schools are excluded from rankings)

Parent Colleges: Vanderbilt and Georgia Tech

Parent Income: > 200,000

So here are my questions to you:

1) Given my current and expected qualifications, what are my chances into Stanford, Duke and Vanderbilt?
2) What can I do in my last two years of HS to improve my chances best?
3) Do those essay topics strike you as powerful enough for a top ranked college?

Thank you all so much.

You seem like a very competitive applicant. Just maintain your grades, and maybe see if you can raise your SAT or ACT (they are good, but why not make them better if you have 2 years). Good luck!
