How to apply to H/EOP

<p>I've applied to several private schools in New York that provides H/EOP.
I know the requirements and I feel that I may be suitable.
I can't find a verification on the Common Application for Columbia or Cornell University for H/EOP. I've searched many websites and they only provide me with information about it.
Can someone help me?</p>

<p>They say: To apply for admission to a Higher Education Opportunity Program (HEOP), request an application from the institution that you are interested in attending and/or check for supplemental forms on CommonApp.org. There are 58 HEOP programs throughout New York State.</p>

<p>But you’re right, they don’t provide you with a place to do it from each institution. I would email the admissions offices of each place.</p>
