How to ask for a teacher recommendation?

I am planning on joining a leadership program, but I need two teacher recommendations. I am not sure how to ask for it over email. Any advice?

  1. Describe the program briefly
  2. Ask if they're comfortable providing a strong recommendation (that way they can back out gracefully if they don't think they can give a strong recommendation)
  3. Mention why you want that teacher to give you the LOR (you developed as a person in their class, their class taught you valuable life skills, the subject they teach is related to the program, etc.)
  4. Tell them what you're looking for in the LOR (briefly provide some content about you to jog their memory)
  5. Provide a deadline that the letter needs to be done by
  6. Make sure you thank them
