<p>Sorry if this question has been answered already, but I'm a little confused on how to get the mid-term report done. Am I supposed to take the same copy to each of my professors and have them sign it or do I give them each a different one? If each professor is supposed to fill it out and give it back to me so I can send it, how does that not violate my waiving my rights to see the information on the form?</p>

<p>You don’t have to waive your rights to seeing the information on any of the forms. It doesn’t say that anywhere. You can even sign saying you won’t ask to see it and still have seen it. Read the words closely.</p>
<p>“Yes, I do waive my right to access, and I understand I will never see this form or any other recommendations submitted BY ME or on my behalf.”</p>
<p>You’re signing off saying that you won’t ask to see it once it’s been submitted.</p>
<p>Ask all your professors to do it on the same form. Bring it from professor to professor.</p>
<p>Usually, the midterm report is due mid/late March after you have your official mid-term scores. Most of the schools I’m looking at want the form by March 28.</p>
