How to make jump from CC to Stanford?

I've been reading this forum a lot lately and I realized how much effort you all put into academics to be competitive for top schools. I'm talking about all the rigorous AP's, high scores on the tests, community service and other tests such as the AMC and other tests I do not remember.

I did horribly in high school with a 2.5 GPA and did no community service, I did sports (swimming and football) but overall, a very weak application that forced me to have to start off at community college.
But would it be possible for me to transfer from a community college to a prestigious school? If so, how?

Buckle down and focus on your studies. If you do so and get top grades, it will be possible to transfer from a CC to a prestigious 4-year university. Stanford may be tough because it doesn’t take as many CC transfers as schools like UCs and USC (assuming you’re at a California CC).

^As far as extra curriculars, community services and standardized tests go, which would better my chances the most? I am so unfamiliar with the process and any help is greatly appreciated
