<p>How can I access the teacher evaluation form so that I can print it and give it to my instructor?</p>
<p>Assuming your school doesn’t use Naviance, you need to invite the instructor online through your common app account. When he/she gets the email invitation, he needs to opt out of online submission. Once that’s done, your account will update and provide you access to customized pdf forms you can print and give the recommender to complete and mail.</p>
<p>My recommender says that she’s opted out the online process, but I don’t see any change to the CommonApp. If she hasn’t successfully opted out, are there instructions that I can show her? Where would I look to see if it had updated?</p>
<p>As soon as she really fully opts out (following the instructions in the email she received when you invited her), your recommenders section in each school on your dashboard should update to give you access to print all the pdf forms. They will be personalized for you with your name, CEEB, CAID, etc. There isn’t anywhere else to obtain the forms.</p>
<p>I made a mistake in the spelling of her first name; do you think that might be contributing to the problem?</p>
<p>Misspelling her name is not an issue. She has the ability to input the correct spelling through her common app account. The link between your app and her rec would not be affected. Go to the assign recommenders section of one of your schools on your dashboard; that’s the place to look for the link to the pdf forms.</p>
