How to review from here?

Hey guys. After taking the SATI for the first official time on May 5, I received the following scores:

M 690
CR 690
W 670

I've come a long way considering that on a Kaplan practice test I took a year ago I only got up to 1690 (w/out Algebra 2). I'm still looking to improve 50-150 points overall, with each subject hopefully in the 700+ range. My test was broken down as follows:

Math: 2 numbers and operations questions wrong, 2 algebra and functions wrong, 1 geometry question omitted, and all data analysis/probability correct (all wrong questions except for 1 were rated difficult)

Reading: 5 difficult sentence completion questions wrong, 3 medium passage based reading questions wrong (the incorrect passage questions were because of faulty time management)

Writing: 8 essay (it went down from an 11 on my last scored test), 4 multiple choice questions wrong

As far as preparation goes, I completed a substantial number of practice tests, mostly in math, in the blue book. I still have yet to open up the Princeton review guide and complete those practice tests as well. I haven't gone over any vocab yet, and all my reviewing so far has been on my own.

I am retaking the test in early November. My current scores were probably fairly lower than they could have been due to a number of factors around the exam date (lack of sleep, high amounts of stress and anxiety, cramming the day before the test). Given this fact, after a summer (and September) of further preparation, are gains of 50, 100, and 150 points realistic for me? If so, how should I go about reviewing in order to do so?
