Hi guys, does anyone know how to submit this form. I just turned in my rankings, and a PDF of the form popped up, but I am not sure if I am supposed to print it, sign, and rescan or somehow type into the computer or what. It doesn’t say. I am really confused. Someone clarify, please!
Print it out and sign it. Get it signed by your parent/guardian and counselor. Scan it and go to where it says Upload Materials. Select Match Agreement Form in the drop-down menu and upload the scanned document.

Wait, the Match Agreement Form isn’t due tomorrow, right? It’s due November 2?
So the only thing due tomorrow is the college list, right?

“If you have not yet submitted your Match Agreement Form, please do so as soon as possible. We will continue to accept the form past the October 13 deadline.”
So they have extended their deadline… at least for a bit.
