<p>It works the same way as the teacher recs: you give the rec-writer a stamped and addressed envelope, and they send it in.</p>

<p>yes...i second FCS: for supp. recs, do you have to use the TEACHER EVALUATION for or do you have the recommender just send in a letter WITHOUT filling out the TEACHER EVALUATION FORM (i assume this latter is correct...)</p>
<p>No, the recommender does not have to use the teacher evaluation form. But s/he should use a letterhead and make clear his/her qualifications for providing the rec..</p>
