<p>Strictly undergrad (and grad, if applicable).</p>
<p>In a fair number of law school applications, I was always asked high school name and date of graduation, and was occasionally asked city or full address. My resume, which was sent along with applications, also went back to junior year of high school, but definitely not grades, test scores, or most clubs...just a few major awards and positions, as recommended by my school's career center. </p>
<p>The one exception might be if you took college classes in high school. You'll be required to report those grades, but they shouldn't hold any weight, to speak of (this issue is debated in a number of recent threads).</p>
