One of my teachers sent in the wrong letter of recommendation form for a certain school. Basically, she sent in a columbia letter of rec on the common app form, not using the proper paperwork. What do you think will happen because of this?

That sucks.

But seriously, how different is the proper paperwork from the common app form?

i would contact columbia and seek their advice. The will probably ask you to send in the proper form.

Columbia probably will not care, but ask anyway. I doubt that this is a "huge mess up." Usually colleges allow teachers to use their forms or to write a letter. The colleges know teachers are very busy, and basically just want a thoughtful recommendation from teachers.
