<p>On the teacher evaluation sheet, where it says IMPORTANT PRIVACY NOTICE in a box, I accidentally left it blank without a yes or no and signature, and already gave the sheet to my teachers. Am I screwed? Was I supposed to choose yes or no? What's going to happen?</p>
