<p>I'll be submitting the IDOC to one college as is required but I have some questions. Any help would be greatly appreciated.</p>
<p>1) For the Verification Worksheet, on the "Household Information" section, on my row (The first one), for the "Name of College", do I put in the name of the college that requires that form? I have not been accepted by it yet, however. </p>
<p>2) For the "Student Non-Tax Filer's Statement" when filling out the "Source of Income", I would be using my "SSA-1099 - Social Security Benefit Statement" form. Would I put in the amount listed in "Benefits Paid in 2012" (This excludes Benefits Repaid to SSA) or "Net Benefits for 2012 (Box 3 mind Box 4)". This same question applies to my "Parent Non-Tax Filer Statement.</p>
<p>Should I attack a "Tax Year 2012 Form 1099-INT Interest Income" form from a bank?</p>
<p>3) Should I attach a document from a trust company that lists trust interest and reports 1099-INT, 1099-OID, and 1099-DIV (This is stated on the top of the document)? </p>
<p>Sorry if some of these questions have obvious answers. I'm not too knowledgeable of such affairs but I'm learning.</p>
