Generally the combined programs are independent of the main admissions. But check with each to make sure.
In my limited experience, acceptance into the combined programs is more challenging/selective. Students are often admitted to the undergrad program but not the combined program up front.

It can vary by school. At Boston University if you are denied for the BA/MD program you will not be considered for regular admission.

That should be clear on each website, it is different by school.
BS/MD programs often do have separate apps because they are generally read by people within medical school admissions, not the undergrad admissions staff.
IME AOs and such have been responding relatively quickly to these types of communications, just make sure you have thoroughly looked on their website before asking.

BS/MD program admission and UG admission are independent. You may get UG but not BS/MD. But few programs after you are not selected for interview of BS/MD programs you need to notify explicitly that you would like to be considered for UG (BU, CWRU).
Many programs they will forward your app, transcripts etc to BS/MD program office but few programs it is a pain since you need to resubmit everything (example Northwestern).
In either case you need to write a separate essay/prompts for BS/MD program and each programs will have their own set of question(s).
At CWRU:
Admission to PPSP is highly competitive. Outstanding students who are not selected for the program still may be considered for undergraduate admission to Case Western Reserve; many choose to enroll in the first-year class.

Most schools allow you to get accepted even if you get rejected to the BSMD, there are some exceptions though. Additionally, for most BSMDs you will have to make a separate application which is usually no additional cost. However, there are those few schools that are different so try and contact the school.
