If you applied to the Natural Sciences Honors Program...

Hello! I am a rising HS senior, and I am starting my application for admission to UT. I will apply to the Natural Sciences Honors Program once I submit my general app.

I know that the NSHP requires a recommendation letter from a math or science teacher. My question is: if I am already planning on getting a rec letter from a science teacher for general admission, could I use that same teacher for the NSHP rec letter, or do they expect you to use someone different?

Also, while I'm on the subject of rec letters: is there a specific form to give to the teacher to fill out for the rec letter, or do they just write it on a blank piece of paper?

Thank you!

Yes! You can ask that teacher to submit your letter of recommendation online as long as they are a science or math teacher. No, there’s no form. Usually teachers have their own template for recommendation letters for students that make it simpler for them to write. The only things you need to give them is a complete resume and envelopes/stamps to mail those letters.
