So I'm looking over the Immunization Record Form, and I see the following in Section A (Required Immunizations), Question 2:
Menningococcal vaccine:Required for all incoming students age 21 and under.

Proof of receipt of a conjugate meningococcal vaccine or a signed waiver declining the vaccination is required for all incoming students age 21 and under.
What constitutes "proof of receipt"?

D's doctor filled in the vaccine dates, is that enough proof?

^When I went, I just took a printed out copy of all of my immunizations and they accepted that.

I did the waiver for that particular vaccine, so I’m not sure if it’s different, but for all of my other vaccines they simply accepted the record that I got from the Health Department.

So, I’d imagine whatever your doctor’s office gives you will work.
