<p>One of the questions on the immunization form is for Hep B blood test for immunity. According to our Dr., everyone gets the 3 doses, but not normally a blood test to check immunity. I called CMU health services and they told me that if you have the 3 doses, then the blood test is not necessary. Thats great, but on the form, it says "All student requirement". I'm probably going to call back and email them both to get in writing, but does anyone know anything about this?</p>

<p>I read that link and it doesn’t say anything about an immunization blood test. Its just that one question on the form. I read it again and I think maybe it means that if you’ve had a blood test to test for immunity, then you have to report it. </p>
<p>Not sure if I can get to a blank form anymore since we filled it out and submitted it. I’ll have my daughter check and see if she can get to a blank form anymore and if so, I’ll try to post.</p>
<p>We printed it out and it does read as if you need both. However, the whole form is confusing. There are other places where alternative ways of satisfying a requirement for immunization are listed separately. Sometimes, it describes the second (or third) method as an alternative, sometimes it just labels both as satisfying the requirement and in this case, it does neither. My daughter just left the Hep B blood test section blank.</p>

<p>The form was particularly annoying - multiple MMR options— and dose drop down menus did NOT match the vaccine record we have for either S or D nor the CDC recommended schedule (the only reason I knew about that schedule is b/c D needed to submit proof for a summer internship).</p>
<p>…so I just entered what S had received and I guess he’ll get an email if something is wrong-- .
I also think a Tuber. skin test-- may only apply to internationals (standard visa requirement to enter US)…it’s NOT routine for children at anytime in their medical care.</p>
<p>I was stunned that no medical history form required a physician signature.</p>
<p>The good news-- did NOT need to resubmit the form for D during sophomore or junior year…

<p>They don’t really care all that much I’ve found. Several kids I know, myself included, only filled those forms out for the first time either at Orientation (See: OH JEEZ WE FORGOT TO SEND THESE IN? Quick, forge the immunization dates!) or upon the first visit to health services where a lackluster employee checks your paper off without really glancing at it.</p>
