Immunization form

<p>Anybody get it yet?</p>

<p>I called the immunization office to check on exactly what is needed and was told a form had been sent out with everything on it. Just wondering if anyone received it yet.</p>

<p>And no, I’m not talking about the little blurb in the Instructions to Appointees.</p>

<p>Yea, I think your talking about the "welcome medical package,"they had a large portion of Q and A at orientation dedicated to it, mine hasn't come yet.</p>

<p>They said it was being mailed out soon and hopefully everyone would receive it pretty close to the first of May. They said they would like to have all that information in by June 1st to allow for getting everything ready for I-day so that it could all go smoother.</p>

<p>I haven't even gotten my second packet yet...let alone an "Immunization Form".</p>

<p>Can someone call and find out exactly what we need? I tried calling today, since the packet didn't include MMR, and was told a list. However, I didn't get a chance to write it down and am afraid to call back.</p>

<p>So, someone call and get a full list (and, of course, post it). I want to start arranging where I'm going to get stabbed.</p>

<p>Son got his immunization packet today. No big deal. They just want you to take the form to your health care provider and have them fill it out with the shots you've received thus far and then get the ones you need and return it by 15 May. There is also a small card enclosed in plastic -- perhaps for blood work....I don't know.</p>

<p>Also paperwork on the following:
-Medical Care at the Academy
-Blister Care
-Injury Free Running
-MAX the AFT/PFT</p>

<p>Oh, and a form to be filled out by your optometrist, only for those of you who wear glasses and/or contact lenses, and sent in prior to 1 June.</p>

All the immunizations you listed in your previous post are included on the AFA immunization form. You'll need to add the Varicella vaccine to it though (chicken pox).</p>

<p>It is recommended that all shots are received at least two weeks in advance of reporting to AFA because of side effects, sore arms, etc. It also states that it takes 4-6 weeks from the time of the immunization to produce immunity from stated diseases -- so get them early on!

<p>do they just want you to tell them what you have by May 15, or to get everything done by May 15? If the former, I'm fine with that. If the latter, I'll have to call them when I receive my form because it's impossible for me to meet that deadline.</p>

<p>Also, I'm not worried about when I get them, so long as my arms aren't sore, because it's either get them before or on I-day, so any time before is obviously advantageous.</p>

<p>Here's what the wording is, verbatim:</p>

<p>Please note that ALL immunization and the Tuberculosis test listed on the form are required for admission. The recommendation is to receive ALL immunizations at least TWO WEEKS PRIOR to inprocessing day. Immunizations have a risk of side effects, and some result in sore arms, fatigue, headaches, and other flu like symptoms. Receiving one or more of these inoculations on the first day of training could result in decreased physical performance during Cadet summer training. It takes 4-6 weeks for an immunization to produce an immune response and protect you from disease. Keep that in mind when scheduling your immunization appointments. </p>

<p>Looks like I'll have to call the Academy. Typical contradiction: get everything at least two weeks prior but turn in the form 6 weeks prior. Makes sense, right, err....</p>

<p>Due to the plethora of information that has been given out and the fact that I received my medical packet in the mail today and assume everyone will receive theirs by tommorow or Saturday, I will wait to place a call USAFA at another time if more information is needed.</p>

<p>The shots it says to get, just for clarification, are:
-Tetanus and Diptheria
-Polio Virus Vaccine
-Measles, Mumps, Rubella (MMR)
-Hepatitus A and B Vaccine or the Twinrix Vaccine
-Meningococcal Vaccine (Menomune or Menactra)
-Varicella (Chicken Pox)
-Tuberculosis Test (IPPD, PPD)</p>
