Immunization Form

<p>Crap. I learn that this has to be turned in BEFORE registration. There is a one semester grace period but that I means I only register for first semester classes at orientation. I have all the necessary information but I still need to get it signed by a public notary. Being Sunday I can't go to the bank and I leave tommorow morning at like 5 AM to hit the road. 2 questions.</p>

<ol>
<li><p>Where in the Minneapolis area can I find a public notary that will do it no problem? </p></li>
<li><p>If it do happen to get it signed in the city, can I turn it at the Boyton Health Clinic no problem so that there will be no complications for when I register?</p></li>
</ol>

i think it’s completely based on self report.
no doctor’s signature is required.</p>

<p>Okay, I understand now. I don’t need to fill out the last part if there is no exemption. No exemption=no need for a notary. Thanks guys.</p>

<p>wait a second. we have to bring an immunization form to orientation? can someone get me up to speed? i didn’t see this written anywhere on any of those orientation brochures.</p>

<p>i received the form twice,
first with the admission letter,
second with the welcome package(after enrolled).
info. is written on the form itself.</p>
