Immunization Records

<p>Hi, I was just wondering when I should send end our immunization records from my doctors office. I know that we have to send them in sometime, but I cannot find the exact date. Does anybody know? </p>

<p>Be sure to have all shots up to date…including tuberculosis test. Also, must have the meningitis shot. If your doctor doesn’t stock it, you can get the shot at the health center at Bama.</p>

<p>i called the actual health office asking the same question. in one place it hinted that you could not register for classes before this was turned in (i.e., summer orientation session)…but the actual health office said you could not move into dorms/housing w/out the form on file (so that is August). best to get it done while on campus, during any visit prior to moving in. also, TB skin test requires 48+ hours, so may be difficult to do while ‘just visiting’.</p>

<p>Thanks! I called my doctor and I only need to get one of the shots. What is the process if I want to turn it in after the shot? Do I just mail it in to the health office?</p>

There is a fax number right on the immunization form: 1-205-348-0630 or you can mail to Student Health Center, Att: Immunizations, PO Box 870360, Tuscaloosa, AL 35487-0360.
doctor needs to have their official office stamp/license # down on the signature line…</p>
