<p>Tetanus/Diphtheria-3 required doses. Tdap (Td with Pertussis for Adolescents known as ADACEL or BOOSTRIX) is required instead of Td as a booster within the past year for all cadets.</p>
<p>Polio 3 doses are required with the last dose given on or after the 4th birthday to complete the series. IPV booster is required within the past year. (Can be OPV or IPV)</p>
<p>MMR 2 doses required or all three vaccines individually with the last dose given on or after the 4th birthday.</p>
<p>Hepatitis A required at least one dose is required before attending the Academy.</p>
<p>Menomune or Menactra are required within the past year. Menactra is the preferred vaccine.</p>
<p>Hepatitis B at least one does is required prior to attending the Academy</p>
<p>Note Indicate if Twintrix, a combination vaccine was used for HAV and HBV immunization.</p>
