Per popular request, a new results thread. Improvements include:
1. No dramatic or controversial preamble.
2. Changed "deferred" choice to "waitlisted" since we are on regular decision now.
3. Added "Deferred EA?" question.
4. Removed "Fee Waiver Used?"--repetitive with the later Income Bracket question.
5. Moved "ACT" option below SAT I to better reflect its nature.
6. Removed "number of apps from your school"--for most this is impossible to tell, also of little importance.
7. Changed the GPA question to purely unweighted--different schools use different weighted systems so listing weighted GPA is of little use to people from other schools.
8. Added "or percentile if rank is unavailable" option to Rank.
9. Added "place score in parenthesis" for AP and IB.
10. Changed vague "other stats" to "Major Awards (USAMO, Intel etc.)"
11. Added "place leadership in parenthesis" for extracurriculars.
12. Removed the second appearance of "Hook".
13. Distinguished State/Country for both domestic and international applicants.
14. Added "Volunteer/Community service".
15. Added "Summer Activities".
16. Added "Other Rec".
17. Other minor corrections/improvements.

Maybe you don't want people to enumerate all of their AP's and scores, as that will take a lot of space. Could you think of a more concise format? One idea is to just list the 5's and then say "and x others"-remember that only 5's count for advanced standing.
