I know there’s a part of the interview when the student asks the interviewer questions about the school. Is it ok to have a list of questions and take notes, or is that bad etiquette? Should it be more off the cuff?

I think general advice is that having a list of questions is good, it shows you have done research and are interested in the school. It is definitely better than not having anything to say.

I think having some questions in mind or on paper will be a positive sign - like isehockey15 said, it shows you've done your research into the school. Make sure you ask questions that are not easily answered from the school's website - questions that it takes a real person to answer, not just factual questions.
