I am applying to four schools through the Common App. I am planning to use one teacher's recommendation for all four schools. Do I just invite him once and then assign his recommendation to all four schools, or do I have to invite him four times?

Invite once, assign to each school

You invite once, he gets one email, he uploads one recommendation. He is not told by the common app what schools his recommendation will go to.

The assigning process is entirely separate from the inviting and recommending process. You can assign him to as many or as few schools as you want.

Can I submit my teacher recommendation by mail instead of online? If so, how do I inform the colleges that I do? Common app wont let me submit unless I fill everything in.

Assuming your school does not use Naviance:

You do still have to invite your recommenders and assign your recommenders to schools electronically through the common app process, even if they will be mailing their recommendations.

Separate from your actions, each recommender has the option (upon receiving the emailed invitation from the common ap) to submit all recommendations for all students during this entire application season either by email or by regular mail. (A recommender cannot elect to submit some recommendations through the common app electronically and others by mail.)

You don’t do anything differently in your common application submission regardless of how your recommender will be submitting the rec. (However, you may want to provide stamped addressed envelopes to the teacher if he or she will be mailing them.)

I only see the invite link under the assign recommender section for each school. How can the invite and assign step be separated?
