<p>What exactly do I need to provide my professors with? Also, is there a common for which they can fill out and photocopy? I'm asking because every school I'm applying to has their own form and I think it would be inconsiderate of me to ask each of my professors to fill out 5 sereperate forms. Please help me. Time is running out. </p>

<p>Maybe your school has a common form, you should ask.
I had the same issue, but because I attend a community college, they have a form for transfers.

<p>Are you using commonapp? They have their own instructor’s evaluation form. What I did was get one form per prof. and give them stamped+addressed envelopes for each school.</p>
<p>So you didn’t even submit LOR’s to the schools? You just gave your professors the common app and they filled out the evalution part of it?</p>
<p>Yes, because I waived my right to see the recommendation, which is really recommended. I filled out everything that I could on the form (addresses, names, etc) and the professor only had to fill out the evaluation form and write a letter recommendation.
My professors just stuck the forms in the envelopes I had given them, and then mailed them.</p>
<p>Do you have a copy of this form? Cause I’m completely lost as to what form you are talking about. Is it the Common APP or a LOR Form?</p>
