If I am a member of the school newspaper
then I rise to be the editor in chief
but in the process:
I am published several times
I am named poet of the month for a major forum
I am a semi finalist for a national poetry contest
Two of my poems were selected to be in an special anthology for America Poets
I am a member of many writing forums
If I won first place in a contest for a major poetry site
won several honorable mentions for that site for contests
Have a writing journal
Have a webpage of all my poetry
Attend poetry slam

Being Published, poet of the month, semi-finalist, in the anthology (especially that one), and being first place in the contest are the hooks. The rest aren't that special because you don't really have to be good to have a journal or be a member of a forum (its like saying, yeah, i am dedicated to University of Blahdeeblah b/c i post on that forum a lot)

If you got published - yes, likely. Remember, these are all "if's." Members of forums won't really matter, and attending poetry slams/having a personal journal don't really matter either. I think you knew the answer to most of these questions before you posted...

** DEFINITION OF A "HOOK" **

Something that an applicant has or has accomplished that few, if any other applicants will have. These should come in the form of sports, music, arts, writing, and not pokemon cards champion...

Actually, the biggest hooks are things many kids have. They are being a legacy, a recruited athlete, a URM etc. You know who competitive these contests on sites are or are not and colleges will know too. Either way, I don't believe they are hooks..

nah they aren't hooks. most kids going into journalism or writing are going to have them and much more.

They don't sound like hooks to me, unless the anthology publication and the poetry contest are well-regarded (i.e. university-endorsed not poetry.com or some other scam). Care to provide a link?

Well, you never know. Incidentally, I know someone who proudly listed "pokemon cards champion" on his application, and is now at Princeton. :)

I would think that anything that is well regarded in the adult/professional world of poetry/writing would be regarded as a hook IF the admission committee realized it. At the very least it is a demonstrated passion.

as long of those are awards , not for Poetry.com
cause thats a joke

"I am published several times"

Published in what publication exactly?

"I am named poet of the month for a major forum"

Questionable; I doubt this'll be a hook since it's from the Internet

"I am a semi finalist for a national poetry contest"

Maybe, so long as it's not from poetry.com or similar sites I guess.

These are more of participation than achievements. I doubt admission officers will have much confidence in Internet poetry sites including those honors you received.
