<p>the counselor signature form for indiana university bloomington says that i have to attach my athletics programs to the counselor form on another piece of paper. do i just type this on the computer or do i need a form for it?</p>
<p>Just type in on the computer–but add a signature line so the counselor can sign it and include it in with the counselor form. Also, be sure to have your name and address on this document so that if it gets separated from the counselor form, it can be matched up to your application file.</p>
