<p>Generally, the schools have a recommendation form — that has some “rate this student” multiple-choice questions as well as some open questions. The GoChoate site has a generic/unbranded recco form for mentors/coaches that aren’t English/Math teachers.</p>
<p>If you are talking about a personal rec, I would exercise good judgement in selecting someone who really knows you well and adds something to your app that the other reccos don’t.</p>
<p>The perfect time to ask for recs was last month. The perfect time to send them is ASAP or with the rest of your application.</p>
<p>@Sevendad: The Choate rec isn’t completely unbranded. At the bottom of the form, right above the signature it says Choate Rosemary Hall in the little blurb. But I’m sure that the admissions office wouldn’t notice it.</p>
