If you get rejected for a Phd, do you get considered for a Masters in the same program or you'd have to apply again or do two applications etc?

Depends on the program. Stanford, Cornell, and Columbia do this while others such as Princeton, Caltech, and Berkeley do not for my field.

Im talking mainly about a Phd in Mathematics.

Really, it just depends upon the school. I had the same question for the schools I applied to, and most said that I had to apply separately. It really just depends on the admissions departments at each school though. I'd just give them a call and inquire about.

As stated, you need to call the schools yourself then.

Just wondering also, if you get denied entry into the PhD program, can you then ask them if you can be considered for their Master's program? I'd assume that it'd be too late for consideration at this point, but just wondering if it's also a case of "Call and ask" since some might.

mcim,

Yup. Call and ask. Definitely. This CANNOT hurt, even if in some situations, it cannot help.
