Letter of Recommendation from High School Teacher

If we're getting a supplemental recommendation from a teacher who taught us in high school, should we only have them write us a letter, or should they fill out the "Teacher Evaluation" or "Instructor Evaluation" from https://www.commonapp.org/commonapp/downloadforms.aspx

I really need an answer by tomorrow as I have to send my teacher the appropriate information. Thank you!

I would ask the school to which you are applying. They might have a preference.

I’ll try that, but first I wanted to see if anyone else had gotten letters from high school teachers and what if they had their teachers fill out any forms as well. Can anyone help?

I second on checking with the school itself.

I applied to 2 schools that allowed recs from hs teachers. One had its own form for the teacher to fill out and didn’t want commonapp’s. The other asked to use commonapp’s evaluations.

I emailed two of my schools yesterday, and I never received a response (besides an automated one which had nothing to do with my question). I’ll try calling tomorrow, but I just wanted to try again and see what you guys thought. Just to clarify, the letter from my high school teacher is completely supplemental (in addition to 2 letters from college instructors). Will the schools be able to match this letter with the rest of my documents if my teacher only sends a letter, or should I have my teacher complete a form as well?

Is it in the FAQs of their transfer page? I found one of my school’s doc there and they specified in the FAQs that they want LOR’s to be sent with the document linked (paper that both prof and I had to sign). Or at least they’ll maybe mention they want or do not want the commonapp supplement.

But in any case, as long as your teacher mentions your name, location, current university, etc. and perhaps your student ID (if they assigned you one), they should be able to match you.

I’ve worked in the offices of my university and have seen LORs and what we do to match them if they don’t have like specific details is we look for name and university they’re transferring from and match it with their information on the system. I assume it’d be the same as other schools but if you’re applying to a bigger school that just may slow down the process.
