Letters, I got "Letters"

Ok, I never thought I would be posting here again once I got into school! So, I am applying for internships and I get reference letters from 3 profs. (I am an anthro major, geology minor.) One is from my advisor, one is from a favorite geo prof and the third is by my french prof, with whom I also did an anthro project. I am in good with all of them.

So I have an app where I am to send *one letter in. My advisor, who is disabled (Parkinson's) gave me a letter NOT on school letterhead. It is also not very well written (all the sentences start with "I") but what he says about me is good. The geo prof's is good, but in the second half of it, he spells my name wrong twice! The french prof's is really good. Probably the best of the three of them. Except, he's a french teacher, and not anthro.

I don't know whose to send. I can't see sending my advisor's, since he didn't write it on letterhead, and they have no way to know that he is disabled. Would it be offending him to ask to put it on letterhead, or to spell my name right? And, I am kind of in a time crunch so they would have to do it in a matter of days. I did ask 6+ weeks ago but they never got online after classes were done (and I emailed him during finals week last semester). I got the french prof's almost right away, he is the best.

The geo prof's doesn't really talk about anthro that much (of course, why I wanted it). but does the repeated misspelling of my name indicate he doesn't know me well enough to spell my name right half the time? It is spelled correctly in the first half of the letter, go figure. And the french one - his would be what I would think my advisor's should have been.

What do I do? Which one do I send? Do I send two? Or do I send one, and a list of all of my references? They all said that the places could contact them.

Thanks -
