<p>i think they mean it can either be submitted w/or without the printable chart teachers fill out, on letterhead or plain stationary, etc. basically just have your teacher submit a letter, even if you used it for the common app (make sure it's addressed correctly). gl!</p>

<p>You can use our Recommendation Form (pdf, 39K), or simply ask that your recommendation be submitted to the Office of Admissions on letterhead, plain paper, stationery, etc.</p>
<p>yes, i haven't looked closely at the common app form since bc had a supplement letter of rec thingy, but as long as you submit one addressed to uw, it will be fine...they probably get every variation under the sun. good luck!</p>
