<p>Yup. Since Rice uses the common app, you use the teacher evaluation form from the common app.</p>

<p>Since Stanford also uses the Common App, you use the teacher evaluation form from the common app website for both of your teachers. There’s no “evaluation 1” or “evaluation 2” thing on the Common App (you’re thinking MIT, which doesn’t use it).</p>
