Loans & Entrance Counseling

So I was on BearFacts looking over the 'Loans' section and ran across this:

What do you have to do?
Complete the on-line Entrance Counseling using the link to the Department of Educations web site in the "Completing the Entrance" section.

After reading the loan information and successfully completing a quiz, you will print a "Rights and Responsibilities Checklist" form.

You will be given three opportunities to pass the quiz. Different questions are asked each time you retake the quiz. You might find it helpful to print out copies of the information that you are unsure about, so you can refer to it while taking the quiz.

If you fail the quiz three times, the site will log you out automatically. You may log back in again and take the quiz another three times. Alternatively, you may prefer to attend one of the in-person sessions conducted by the Loans and Receivables Office. The dates and times of the sessions are available at <a href="http://fbs.berkeley.edu/LRO/loans/entrloan.htm%5B/url%5D">http://fbs.berkeley.edu/LRO/loans/entrloan.htm</a></p>

Complete the form with a black ink pen. Be sure to check off all of the boxes as you read them, sign your name and provide your social security number.

Mail the form to:
University of California
Loans and Receivables Office
Attn: Entrance Loan Counseling
192 University Hall #1110
Berkeley, CA 94720-1110
Faxed forms are not acceptable. We must have the original on file.

I passed the quiz a while ago, but I didn't know we had to mail anything in. I went back to the site to find this checklist, but at the "Congrats! You've completed your requirement!" page it only gives me the option to click on a form where rights and responsibilities are bulleted (no place to 'check off' stuff). There's also no place to sign... should I just sign on the white space and mail that in? Or is this a question that requires me to call someone and be put on hold for 45 minutes?

I did it for you ha. I was put on hold for approximately 25 minutes. The lady said we don't have to mail anything in because we notified the college that our results should be sent to so I think we are good for the most part.

We just need to wait for it to transfer through and she said possibly next week. Same thing with the cal bears scholarship money. Next week.
