<p>How should I let a LOR from my highschool headmaster be written on. On the normal common app or would a normal letter be enough? </p>

<p>Is this a required Instructor Evaluation or a supplemental LOR?</p>
<p>Required LORs should use the IE form. </p>
<p>Supplemental LORs can be just a letter, preferably on the HS letterhead and with identifying information (your full name, DOB, etc.) so that schools can match the LOR with your application.</p>
