I checked my application materials online and they haven't received my secondary school report yet. My counselor sent it in early December so that was almost 2 months ago. Oddly, my teacher sent his recommendation a week earlier and it has already been received.

Should I be worried that the secondary school report got lost in the mail?

Eslite, when my DD applied during the SCEA round, one of her recs was lost in the mail - the USPS does not deliver at times! I called Stanford to see what we should do, and to make a long story short, her GC ended up faxing a copy of the rec. If I were you, I would call them to see what they want you to do.

Thanks. I'll definitely call them tomorrow night (from Malaysia so 16 hours time diff there).

My teacher sent it registered and I've just checked the status. It has definitely left the country but has yet to reach the States. Really expecting the worse. :/

I called them. They asked me to wait while they sort out everything since if I resend now they'll impose a penalty (never knew that!).

Anyone know what details you should include when you resend documents? I remember it is name and date of birth and another which I couldn't remember.

My daughter called yesterday about a missing rec and they just told her to have her GC fax it in with a cover letter. Nothing about a penalty. What could that possible be?

No idea. I called and told them most likely its lost in the mail. The person asked me to wait until they officially notify me about it (which should be around next week) and when I asked to resend the things they say I need to provide proof that the mail is lost otherwise there will be a penalty. She also said that if I wait until the official notification, no penalty will be imposed.

Have the guidance counselor fax the missing report asap. Be sure to have the school write her Stanford application ID # on the report, as well as her SS#, if applicable. She was issued a ID # when she opened her application online. Who ever told you there would be a penalty is BSing you. Stanford admissions screwed up big time last year- lost S's transcript, as well as letter of recommendation letter, did not find them until SCEA deadline had passed. I suggest you be proactive and stay on top of things.
