I encourage the OP to send in the appeal but also have a plan B. To never have met the standard in the first four semesters is unfortunate, and I also think that extra year was generous.
It also might be helpful to know how many students lose their scholarships. If it is rare, the school might be more likely to make an exception. If it is common, I think the school is going to apply the rules uniformly.
A lot of Florida students lose the Bright Futures renewal. GPA not high enough, didn’t take enough credits (often caused by dropping a class and thus dropping below 12 credits), not finishing with a the number of semesters allowed. They typically give one year of grace, but then pull the award.
