If a college doesn’t have a musicology major, I’d probably apply as a music performance one/music history one. I prepared my music supplements, and my teachers offered support for LOR + feedback!

Excellent. If you haven’t arranged a meeting yet at Vanderbilt you should reach out. They have a bachelor of musical arts major that allows for a performance major and easily accommodated a second major. You would also be competitive for their full scholarship but deadline is 12/1

Unfortunately, my school didn’t offer the best amount of support when I was ill. So I’m hoping to somehow explain that on my college additional info section.
Different schools have different LOR requirements. You’ll need to check with each school.

They do trial lessons for a small fee. You can meet the teacher and find out about the studio and see if it is a good fit for you. They do zoom if traveling before auditions is not reasonable.

OP: Wouldn’t music performance be determined by an audition ?
At a BM program requiring auditions, this is absolutely true.
Then OP should consider adding Northwestern University’s Bienen School of music.
Perhaps others can comment, but on this list, I really don’t see a lot of strong music performance BM programs. Vanderbilt, UMich, BU, CMU have good music performance programs. The rest of this list just looks like tippy top schools.
So to @luciapolporo could you please clarify whether you are looking for music performance programs? If so, I’m sure we can add others. Northwestern is a good addition. Another could be Rice.
For less rigid academically, I believe SMU and Syracuse are both very good in music. In other words, better odds of getting in than the current list from an academic, not talent, POV.
If this kid does an auditioned music performance application, the academics he has are just fine. It’s the audition that will determine his acceptance.
