As others have noted, you need actual likelies or better safeties unless you truly would be fine with Rutgers.
While your reaches are all super reaches for everyone and you shouldn’t get your hopes up, I think you have a good shot and some of them. Based on all the awards and achievements, you probably could do well at the audition programs and being great at music and wanting to major in it is often a minor hook at some of those other reaches that don’t have an audition (assuming you produce a great music supplement). Columbia loves music kids, and it sounds like you already made a good impression there.
As others noted, its really on your counselor to help make the case for why sophomore year was a GPA fluke due to your health issues and why junior and senior so far are more representative. I wouldn’t worry about it too much – the combination of your music ECs and test scores will get you into the consider at most schools and your GPA likely won’t be the deal breaker.
Note that for the UC’s on your list, they only look at Sophomore and Junior year GPA and they strip out all + and - grades – so a B+ is a B and and A- is an A, etc. This will hurt your GPA there a bit. For Out of State applicants like you, they also will not include any of your non-AP or Dual Enrollment honors courses in their weighted GPA (only in-state applicants can count honors courses). They calculate the weighted GPA two ways. The “capped version” should you max out on AP and DE credits anyway, but for the “uncapped” the lack of counting honors can hurt. Only a few of the UC’s state they use the uncapped, including UCLA.
Note for USC, as a National Merit Semifinalists you are eligible if accepted for a Presidential Scholarship that would be half-tuition merit (not related to financial status). But you need to apply EA – you remain eligible even if you are deferred. Since it’s EA, that only becomes a problem for some other REA/SCEA schools, and some of them (like Stanford) make an exception when the EA is for scholarships. (Speaking of Stanford, it’s not on your list so probably not an issue but note both their EA and RD application dates are earlier than the regular date if you are submitting a music supplement.)
Seems like you are “shotgunning” the Ivys. I assume you’ve sorted with your parents that they will front the cost of so many application fees? They are very different schools and locations. The major also have an ED option so you may want to think about whether you have a strong first choice (if this was already covered in one of the replies above, sorry I missed it).
