Can we just do now or wait until college send us some forms and then go to our family doctor to do it?</p>

Dartmouth will send you a medical form over the summer which will have to be filled out my your D's doctor and submitted back to the school. In addition they will request a meningitis vaccine or signed waiver. So you might as well wait until you get the form.</p>
<p>In addition, the school will send you a health insurance waiver where you will have to show that you h ave adequate health insurance coverage for your daughter (or purchase the student health insurance plan.</p>
