<p>It certainly would be nice if insurance companies offered standardized HIPAA disclosure forms allowing a single point of signing for discussion of medical issues (separate forms for POA!) with a defined list of family members for all providers covered in the plan. It would save many providers a lot of time if they knew upon check-in that they didn't have to get that silly form done as well as the patients who have to endure one more piece of writer's cramp.</p>
<p>Probably some legal issues that prevent it.</p>
