Mid-Year Report Form

S was accepted ED and his acceptance letter included a reminder for him to have his counselor submit his Candidate Mid-Year High School Grade Report but I am having trouble locating the form. Any ideas on where I can find this?

I see that it’s listed in required documents but I don’t see a link anywhere on Common app. Maybe I’m not looking in the right place?
