<p>No. Only if something changed e.g. your rank, or your counselor's opinion of you, then your counselor has to write a new one. I believe it says something like this on the mid-year report.
I have no idea when it's due. I think it depends on the school, some don't care at all, others specifically request the midyear results as soon as they become available. I doubt that there is some fixed deadline, especially since the 1st semester might end in late december for some, and in late january for others.</p>

<p>They fill out the Midyear Report form exactly as directed. At the top of the second page of the Midyear Report form, it gives directions about what to fill in and what to not fill in for that page.</p>
