<p>There may be specific mid year forms from some schools, but not every school.</p>

on the common application website it states which schools require what.
some of them want a full report, some don't. some need two teacher recommendations and some need one.
other than that it is all the common app forms unless stated otherwise in the supplement section of the college on commonapp (such as Pomona which requires 2 other supplements to be submitted off site)
