<p>Is there a form for sending in midyear reports like the common app one? I looked in the forms area of the website and I don't see one.</p>
<p>i remember i saw on the UMICH webpage
it said unless requested, you dont have to submit any other application forms such as mid-year (but mid-year report is required if you are defered)</p>
