<p>What high school form? You mean a new counselor report? I don't think that is necessary, unless your rank has changed, or something notable that is not listed on your transcript has occurred.</p>

<p>You are not required to submit mid-year school reports unless you are notified by the Office of Undergraduate Admissions that we will need these grades. Mid-year school reports are recommended for applicants who have received a deferred (postponed) decision.</p>
